The most common cause is incorrect tension — a loose rubber track walks off the idler during turns, especially when reversing or working across a slope. Check sag against the manual and re-tension via the grease adjuster. Other causes: worn or stretched track carcass, worn sprocket teeth that no longer engage the drive lugs cleanly, worn idler or roller flanges, mud and stones packed inside the frame, and driving technique with fast counter-rotations on rough ground. Inspect the metal cores of the track — if several are exposed or missing, the track needs replacing. If a new, correctly tensioned track still derails, measure sprocket and idler wear before blaming the track.
